Druid Meteoric Items?

I have a druid alt that I'm leveling to dual log with my main, and I would appreciate it if someone could tell me what items are needed for the meteoric upgrade. Not the rems and tabs, but the goblets, ingots, gems, etc. Thanks in advance!
