The idea behind having health is that mages and druids usually don't use auto attack, so defense would be wasted there.
If a warrior begins casting giant swing on a player moving on an 80% mount, the giant swing will complete no matter where the player is. If the warrior decides to auto attack the moving player, the auto attack will not complete and the warrior will instead sluggishly follow the moving player. The same applies for rogues. That's why warriors and rogues typically spam skills in duels against runners.
Only a ranger should be able to hit you at long range with auto attacks, in which case the defense would help. So overall, health is effective vs 4 out of 5 classes, while defense is more effective vs 1 class (rangers). That is why the health brooch is recommended.
However both brooches are totally fine to use, or any other decent misc charm that you have; as mentioned before, the really important part of the build lies in having both 1 million gold mounts, 2 cloaking braces, and maxed skill evasion abilities. Everything else is pretty much optional.
